Skilled Nursing, Rehab & Transition to Home Services

Our health care professionals include nurses, therapists and aides. They work under the direction of our medical director. Together, they help patients who have had an illness, accident or surgery return to their maximum level of independence and function. Our private rehab suites have flat-screen televisions and access to Wi-Fi. They are designed to keep patients connected to their loved ones in the community while they rehab to return home. We work with patients with sub-acute medical needs, in addition to those needing a structured therapy program. Our Transition to Home clients include people with a variety of needs such as stroke, cardiac, pulmonary, and orthopedic rehab.

Assisted Living at Hilty Home

If a loved one is no longer able to live safely at home, our Assisted Living can provide a solution. We encourage independence while supporting residents with the services they can no longer perform on their own. Our apartments are available in single or double units. They are rented on a monthly basis with no long-term commitment or lease required. Hilty Home is also an approved provider of the Medicaid Waiver program for those who qualify for this financial assistance.

Each apartment consists of a living room/bedroom combination, a kitchenette, and a large private bath. The bathroom is equipped with grab bars, an emergency call system and a walk-in shower.

Mennonite Home Health & Senior Services Celebrates 20 Years

Twenty years ago health care choices for seniors who were in need of care were
somewhat limited to either a hospital stay or going to a nursing home.
Kathy Rode, a nurse at the Mennonite Memorial Home in Bluffton, saw that many
of them could stay at home with just a little assistance. "We wanted to
offer a third option in the way of home health services, where nurses would
make visits in people's homes to help them recoup after surgeries or if they
needed further assistance." recalls Rode, who is the founding Director of
Mennonite Home Health & Senior Services. "We decided that this would
be an excellent service for the Mennonite Home to offer the elders in this
community and so began the journey of the last 20 years."

The nurses began offering in-home services such as;
skilled nursing care, medicine supervision, post-surgical care, physical
assessments, therapy services and home evaluations. Mennonite Home Health
& Senior Services also had nursing assistants who went to client's homes to
give support in the form of personal care, bathing and other needs. Over
the last 20 years Mennonite Home Health & Senior Services has served
hundreds of clients in 4 counties and has seen the size of their staff
double. "We have made wonderful friends, shared many memories and
enjoyed giving excellent care," says Rode. "Many children don't live in
the area and they rely on us to provide that weekly interaction and
communication with other health professionals about the needs of their loved
ones. It is a very rewarding job." Rode has worked for MHCO for 34
years and has enjoyed her long career in serving elders.
