One of the highest confirmations of quality service is for a Home Health agency to receive a deficiency-free survey from the Ohio Department of Health. Mennonite Home Health & Senior Services in Bluffton is pleased to have accomplished just that on the 2021 state survey, which took place in May. Teresa Lewallen, Director of Mennonite Home Health & Senior Services, stated that “Survey time is always stressful, however it helps to keep us focused on our goal which is to provide the best care that we can for our patients. I would like to thank our team for their continued commitment to quality care.” The survey is conducted annually by the Ohio Department of Health and includes a checklist of areas and categories to examine. Some of the categories include meeting with clients, reviewing medical records, and observing care.

Drive in Bluffton, to the Mennonite Memorial Home building at 410 W. Elm Street, Bluffton. This office can be accessed from the main entrance of the Mennonite Memorial Home, or by a separate entrance accessed by the parking lot behind the Mennonite Memorial Home.
