Next Tuesday, June 15th , is the monthly COVID-19 vaccine clinic. These clinics continue on the 3rd Tuesday of each month.
Home Town Pharmacy will provide the Moderna vaccine to interested staff and residents.
If you prefer a one-dose vaccine, different brand, or ability to schedule, please reach out to your campus Director of Nursing for information about getting the vaccine of your choice. There is no cost to you for any vaccine you choose to receive, from any local source. If you wish to receive the vaccine next Tuesday the 15th, please get a consent form from your campus office and return it by Friday, June 11 to Mennonite Memorial Home. You can also download the Vaccine Consent Form here.
